Surgery for uterine fibroids
Removal of fibroids with procedures that allow the uterus to be preserved.
What are uterine fibroids?
Uterine fibroids are benign tumours that develop in the muscle of the uterus. They are very common in women of childbearing age and can vary in size, number and location.
Surgery to remove uterine fibroids in Mallorca
When the patient wishes to preserve her uterus, whether for reproductive or personal reasons, there are specific surgical techniques that allow the fibroids to be removed without the need for a hysterectomy.
Treatments
Main indications:
- Miomas intramurales o subserosos.
- Moderate to large size.
- Multiple fibroids, up to 3-4 in number.
Advantages:
- Preserves the uterus.
- Less postoperative pain.
- Reduced bleeding.
- Faster recovery (1 to 2 weeks).
- Lower risk of adhesions compared to open surgery.
Limitations:
It may not always be possible if the fibroids are very large or numerous.
Duration and Recovery:
- Surgery duration: 1.5 to 3 hours.
- Hospitalization: 1-2 days.
- Return to daily activities: 1-3 weeks.
Outpatient procedure performed by inserting a hysteroscope (a thin camera) through the vagina and cervix into the uterus, with no need for external incisions. It is used to resect fibroids that protrude into the uterine cavity.
Main indications:
- Submucous fibroids (FIGO type 0, 1, and some type 2).
- Abnormal uterine bleeding.
- Infertility related to distortion of the endometrial cavity.
Advantages:
- Outpatient technique or short hospital stay.
- Very rapid recovery (2-5 days).
- Completely preserves the uterus.
- Leaves no external scars.
Limitations:
Only useful for fibroids that protrude into the uterine cavity. May require more than one procedure if the fibroids are large or multiple.
Duration and Recovery:
- Surgery duration: 30-60 minutes.
- Hospitalization: discharge on the same day.
- Return to daily activities: the next day.
When fibroids are very large, numerous, or located in areas that make access difficult via minimally invasive techniques, it may be necessary to perform a laparotomy, meaning open surgery through an abdominal incision.
Main indications:
- Patients with large fibroids that completely distort the uterus.
- Need for complex reconstruction of the uterine cavity (especially in women who wish to preserve fertility).
- Failure or contraindication of laparoscopic or hysteroscopic techniques.
Advantages:
- Allows for direct and complete visualization of the uterus.
- Facilitates precise resection of fibroids that are difficult to access via other routes.
- Enables meticulous repair of the myometrium and endometrial cavity.
Considerations to take into account before the operation:
- Longer recovery time (4 to 6 weeks).
- Higher risk of bleeding and adhesion formation.
- Requires longer hospitalization (3 to 5 days).
- It may involve more postoperative pain than minimally invasive techniques.
When uterine fibroids are multiple, large, or cause severe symptoms, a hysterectomy (removal of the uterus) may be the best option. Several surgical techniques exist, but today, vNOTES surgery stands out as one of the most advanced and beneficial for the patient.
We are the specialists in the Balearic Islands with the most experience in gynecological surgery using the vNOTES technique.
Vaginal Natural Orifice Transluminal Endoscopic Surgery (vNOTES)
The least invasive technique.
What is it?
It is an endoscopic surgery performed through the vagina, without external incisions. A camera and surgical instruments are inserted through the vaginal canal to perform the hysterectomy.
Notable advantages:
- No visible scars (there are no cuts in the abdomen).
- Less postoperative pain.
- Faster recovery (in many cases, less than 2 weeks).
- Lower risk of infections and complications.
- High surgical precision thanks to endoscopic vision.
- Ideal for patients who want a discreet, effective surgery with minimal physical aggression.
Indications:
- Multiple or symptomatic fibroids.
- Women who do not wish to preserve the uterus.
- Cases where rapid recovery and aesthetic results are sought.
Laparoscopic hysterectomy
The standard minimally invasive technique. It is performed through small incisions in the abdomen. A camera (laparoscope) is inserted, and the uterus is removed vaginally or through one of the incisions.
Advantages:
- Menor dolor que la cirugía abierta.
- Recovery in 2 to 4 weeks.
- Good visualisation during surgery.
Limitations compared to VNOTES:
- Requires abdominal incisions.
- It may leave visible scars.
- Recovery somewhat slower than expected.
Laparotomy hysterectomy
The traditional open technique. It is performed through a wide abdominal incision. It is used in complex cases, such as very large fibroids or when anatomical reconstruction is required.
Advantages:
- It allows access to the entire uterus and pelvic organs.
- Useful in cases of difficult surgery.
Limitations:
- Greater postoperative pain.
- Longer recovery (4 to 6 weeks).
- Higher risk of adhesions and complications.
- Visible abdominal scar.
